How to Request your Appointment at NCC

• - Send an Email to nccip@ncc.re.kr
• - Once your request accepted, our Referral team will contact you to collect the following information
  •    ㆍDemographic Information required for patient management system
  •    ㆍMedical Letter to address your current health issues (diagnosis)
  •    ㆍWritten Pathology Reports and Biopsy Slides
  •    ㆍImage files of MRI/CT Scan/Ultrasound and their Reviews
  •    ㆍMedication information and Dosage
• ※ All Medical Records must be in English.
• ※ Submit the soft copies via drop box or email.
• ※ Your Medical History should be provided before scheduling the first appointment and making an estimate.
